STUDENT ENROLLMENT APPLICATION CHECKLIST

Please make sure that your application packet includes all of the following items:

• Completed Student Application
• Copy of Birth Certificate
• Copy of Immunization records
• Report of Health Exam (for Kindergarten and 1^st grade students only)
• Completed Oral Health Assessment (for Kindergarten and 1^st grade students only)
• Copy of Proof of Residency: Property tax statement or two current utility bills
• Copy of Individualized Education Plan (for students with history of Special Education only)* IF APPLICABLE
• Copy of Suspension Notice or Expulsion Notification (expelled students only). Enrollment pending administrative approval.** IF APPLICABLE

FAQ’s

1.     Must I include an original copy of my child’s birth certificate?

·         No, a copy of the original will be sufficient.

2.     Where can I get a copy of my child’s immunization records?

·         You can obtain a copy of immunization records from your child’s physician.

3.     I can’t find my child’s birth certificate or immunization records, what do I do?

·         Contact the last school your child was enrolled in and request a copy of these documents.

4.     Where do I get the Report of Health Exam filled out?

·         Take the Report of Health Exam to your child’s physician or a local clinic and they will be able to complete an exam and fill out the paperwork.

5.     My child hasn’t had an Oral Health Exam and I can’t get them to the dentist …

·         On the back side of the Oral Health Exam form there is a waiver, fill out the waiver and check the box that reflects your situation.

6.     I live with someone and none of the bills are in my name …

·         Obtain the two utility bills from the address you currently reside at along with a letter from the person you are living with stating that you and your child(ren) are living with them and you have no mail and/or bills in your name.

7.     I could only find one proof of residency, is that ok?

·         No, we must have two. You can use any official document that has been mailed to your current address with your name and address on it. Examples: Phone bill, cell phone bill, utility bill, child support checks, car insurance statements, bank statements, rental agreement, etc.

8.     What is an Individualized Education Plan?

·         An Individualized Education Plan (IEP) or Program is a mandated part of the education of people who have been deemed to have special education needs.  This plan would have been created by your child’s school. If you do not have a copy of your child’s IEP you can request a copy from the last school your child attended.

9.     I turned in my application, now what?

·         Your child’s application will be processed and we will call you if we have any questions or concerns. Every parent/guardian will be contacted the week of May 24, 2010 to inform you of your enrollment status and notify you of any additional school updates.
